fix(cli): clarify default MCP tools

Browse Source 
Separate the default streamed MCP surface from advanced and CLI-callable tools in --help output. This keeps help aligned with tool_mode=streamlined, get_code, and _hidden_tools without changing dispatch, schemas, config, or handlers.\n\nValidated with production build, rendered --help, source-safety, and diff check.
