Fix image input and add provider card ordering and model labels

Resolve model catalog text-only gating, migrate verified legacy catalogs, and clarify the Codex Ultra setting. Persist cross-source provider card order with pointer and keyboard controls and display selected model names. Refs #61 , #62 , #63 , #64 , #65 .
