Chris Tate 32745afc07 Bind generic arrow captures before parameter defaults, and count var redeclarations as writes
- A generic arrow instance binds its trailing capture locals before it
lowers the declared parameters, so a parameter default that reads an
immutable enclosing binding resolves it instead of failing with SC1090.
  The captures stay trailing parameters.
- A binding is written when one of its own declarations writes it: a
  `var` in a for-in or for-of head (every iteration), or a redeclaration
  that initializes (`var x = 1; ... var x = 2;`, a parameter or function
declaration that a `var` redeclares). Such a binding is now shared with
generic instances through the environment form, so a closure an
instance returns reads the current value instead of a copy.
