Andrew Hundt d711def474 fix(cli): derive lifecycle guidance from effective config

Read tool_mode, auto_index, auto_index_limit, auto_index_deps, and auto_dep_limit without creating _config.db in ha_load_guidance_config at src/cli/hook_augment.c:1153.

Route Gemini SessionStart through the event-compatible hook-augment command in cbm_upsert_gemini_session_hooks at src/cli/cli.c:4515; retain the released static command only for owned-entry cleanup.

Share CBM_DEFAULT_AUTO_INDEX_LIMIT from src/cli/cli.h:390 and document streamlined reveal, classic source retrieval, and dependency caps in README.md and docs/CONFIGURATION.md.

Verify tests/test_cli.c:4169 plus cli 238, mcp 272, tool_consolidation 113, and input_validation 56 passed.
