Drew Newberry 5600b807fa feat(gateway): support ssh:// endpoint shorthand for remote gateways 
`gateway add ssh://user@host:port` is now shorthand for
`gateway add https://host:port --remote user@host`. The SSH
destination and HTTPS endpoint are parsed from the URL automatically.
